This form is used to establish new companion animal owners with the practice. It asks basic information for the owner as well as the pet(s).
New Client Form – Large Animals
This form is used to establish new large animal (cattle, camelid, goat, sheep or horse) owners with the practice. It asks basic information for the owner.
Farm Visit Request Form
This form is used to request a farm visit. Clients fill in the animals they wish to have seen and the procedures they are requesting for each animal. The form is then printed at the office and is brought to the appointment by the veterinarian, who uses it for note taking. The form is then filed at the office for future reference.
Fecal Sample Submission Form
This form is requested when submitting a fecal sample for evaluation. It asks valuable information which can help to determine treatment recommendations, if needed.
Reproductive History Questionnaire – Female
This form is used when breeding issues have arisen with a female alpaca. We ask that this form be filled out in its entirety, which will assist the veterinarian in developing a diagnostic and treatment plan.
Reproductive History Questionnaire – Male
This form is used when breeding issues have arisen with a male alpaca. We ask that this form be filled out in its entirety, which will assist the veterinarian in developing a diagnostic and treatment plan.
This form can be used by breeders for record keeping or can be used to supplement the Reproductive History Questionnaire. It tracks the breeding activities of an animal for an entire year.
Cria Birth Record
This form was developed for breeders to assist in record keeping of cria births. One record for each cria. It lists all of the recommended information and activities surrounding the birth of a cria.
Pre-Purchase Exam Form
This form is used during the examination of an animal prior to its purchase.
Sire Breeding Chart
This form is used to track a stud’s breeding activities for one month, with all of the females he is servicing. Subsequent forms are printed out for additional months. This form can be used for on farm record-keeping, or as an aid in diagnosing a problem if needed.
Anesthesia & Surgery Consent Form
This form is used when an animal is presented to the hospital for anesthesia and surgery. It provides information and options and obtains consent for the procedure.
Llama & Alpaca Cria Care Sheet
This is a step by step handout on the care of a newborn camelid. It could also be used as a guideline for other small ruminants (goats and sheep).
Milk Supplementation Chart
This chart provides pre-calculated amounts, based on the cria’s weight, for milk supplementation. It is used in the event that a cria loses its dam, the dam is too ill to care for the cria, or the dam just needs a little assistance in providing enough milk to allow for the cria to grow and thrive.
Pellet Gruel Supplementation Chart
This chart provides a guideline, based on weight, on how much needs to be fed on a daily basis. It is used for sick or debilitated animals, or sometimes as a starter to get an animal eating a pelleted supplement.

This provides a list of items that PSVG recommends be kept on hand for anyone breeding and birthing small ruminants.
First Aid Kit – Alpacas & Small Ruminants
This provides a list of items (supplies and drugs) that PSVG recommends be kept on hand for camelids and small ruminants.
